120-minute facilitated team meeting starting with the adult life domains of employment, post-secondary education, independent living, social-emotional life, and self-determination; moves through medium-term goal-setting and ends with concrete action steps. Includes meeting prep and facilitation, and generation of post-session written and visual summaries. (assumes about 4 hours of work)
